How We Protect Your Information via Established Security Procedures
We are committed to the security of your medical, financial and personal information. Security measures are in place to protect the loss, misuse, and alteration of the information stored in our electronic medical record (EMR) system.

We are required by law to take all reasonable steps to secure and safeguard this information.

We are required by law to notify individuals if their personal information has been misused or involved in a security breach.

We Limit Employee Access To Information
We limit employee access to information about our patients, to those with a business reason to know such information. We assign each individual with a business need to access your information with a user id and password. Strong passwords requirements are enforced in the system. Employees are informed of the importance of maintaining the confidentiality of our patients' information and on the content of our Privacy and Security Policies.

We Use Technology
We use Security systems that are structured to deter and prevent hackers and others from accessing information you provide to us.

• We use firewall systems to prohibit unauthorized access and we use intrusion detection software to identify unauthorized attempts to access information or otherwise cause damage.
• We encrypt sensitive information that is transmitted over the Internet.
• We build information security into our systems and networks using only the latest internationally recognized security standards, regulations, and industry-based ’best practices’.
• We perform data backup periodically to prevent loss of information.
• The information and hardware are stored in a data center and access to the facility is controlled 24 hours a day. All of our informational and information technology systems are in a secure environment that protects your information.
• We maintain and grant access to our patients' information only in accordance with our internal security standards.

Due to the nature of Internet communications and evolving technologies, we cannot provide, and we disclaim, assurance that the information you provide us will remain free from loss, misuse, or alteration by third parties who, despite our efforts, obtain unauthorized access.
